What to expect

The visit, end to end.

A full annual physical takes 30–45 minutes. We do the talking and reviewing by video, and coordinate any in-person components — labs, vaccines, physical exam touchpoints — through NYC partners.

Before your visit

  • Complete a 10-minute health questionnaire in your portal
  • If you have any home BP cuff or scale, bring readings
  • List of current medications, supplements, and allergies
  • Any forms that need filling (we'll upload them)

During your visit (30–45 min)

  • Detailed history review with your NY-licensed PCP
  • Mental and emotional wellness check-in
  • Review of vitals (with you on camera if needed)
  • Updates to vaccinations, screenings, and risk assessments
  • Goal-setting for the year

After your visit

  • Lab orders sent to Quest or LabCorp (often $0 with Medicaid)
  • Any referrals (mammogram, colonoscopy, specialists) coordinated
  • Completed forms uploaded to your portal
  • Care plan summary sent to your portal

Common checkup questions.

Can an annual physical really happen by video?
Yes — for most adults, the comprehensive review, history-taking, and care planning happen by video. Hands-on exam components (heart and lung listening, pelvic exam, certain palpations) are scheduled at a partner clinic if clinically needed. Many NY Medicaid plans recognize and pay for this hybrid model.
Will you sign my child's NYC DOE school form?
Yes. Our pediatricians complete and sign the standard NYC Department of Education school physical and immunization forms during the visit. The signed form is uploaded to your portal — you can download or forward to the school directly.
How much does an annual physical cost?
For NY Medicaid Managed Care members (Healthfirst, Fidelis, MetroPlus, EmblemHealth, MVP, Anthem HealthPlus, Molina, UnitedHealthcare Community Plan), preventative annual physicals are $0. For commercial insurance, your plan likely covers preventative care fully under ACA rules.
Do I need to fast for lab work?
It depends on which labs are ordered. If we order a lipid panel or glucose test, fasting 8–12 hours is recommended. Your PCP will tell you exactly what to do during the visit.
Can I get vaccines through telehealth?
Vaccines themselves require in-person administration. We order the appropriate vaccines (flu, COVID, Tdap, HPV, shingles, etc.) and send you to a NYC partner pharmacy or clinic to receive them — usually at $0 with Medicaid.
