Does NY Medicaid cover chronic care?
Yes — and it covers it well. NY Medicaid Managed Care plans cover the visits at $0 for most members, and they cover home monitoring devices (BP cuffs, glucometers, CGMs in many cases), lab work, and most chronic disease medications with little or no copay.
Do I lose my current specialist if I join your program?
Not at all. Many of our patients keep their cardiologist, endocrinologist, or nephrologist. We coordinate with them — we don't try to replace them. Specialists often welcome having a PCP-level coordinator handle the day-to-day.
What devices do you provide?
Depending on your condition and plan: home blood pressure cuffs, glucometers, continuous glucose monitors (CGMs), connected scales. Most are mailed to you and covered by your plan. We help you set them up.
How often will I have visits?
Typical cadence: monthly for the first 3 months while we get things stable, then quarterly for ongoing management. Plus an annual comprehensive review. More frequent if needed; less frequent if you're rock-solid.
Can I message my clinician between visits?
Yes — portal messages get answered within 1 business day for routine clinical questions. Urgent issues (severe BP spike, blood sugar crisis) get same-day response or on-call support.
