Obesity is a chronic medical condition with multiple drivers — genetic, metabolic, behavioral, environmental. The new generation of GLP-1 medications (semaglutide, tirzepatide) is genuinely transformative for many patients. But they aren't right for everyone, they aren't a substitute for lifestyle change, and they have specific risks. We approach this seriously.

Who's a candidate for GLP-1 weight medications

  • BMI ≥ 30, or
  • BMI ≥ 27 with at least one weight-related condition (diabetes, hypertension, sleep apnea, cardiovascular disease, fatty liver)
  • No contraindications (pregnancy, history of medullary thyroid cancer, MEN2, certain GI conditions)
  • Willing to commit to lifestyle change alongside medication

What's covered by insurance

  • For diabetes: Ozempic, Mounjaro, Rybelsus are covered by most NY Medicaid plans
  • For weight loss only: Wegovy and Zepbound coverage varies; many plans require prior auth and may not cover. We check before prescribing.
  • Older options: phentermine, topiramate, naltrexone-bupropion (Contrave) are sometimes covered when GLP-1s aren't

Will Medicaid cover Ozempic or Mounjaro for weight loss?
If you have diabetes, yes — most NY Medicaid plans cover Ozempic/Mounjaro for diabetes management. For weight loss without diabetes (Wegovy/Zepbound), coverage is variable and often requires prior authorization. We check your specific plan before prescribing and handle the prior auth paperwork.
Are GLP-1 medications safe?
They have a strong safety record with millions of patient-years of use. Most side effects (nausea, constipation) are manageable with slow dose titration. There are specific contraindications (history of medullary thyroid cancer, MEN2, severe gastroparesis, active pancreatitis history). We screen carefully before prescribing.
What if I stop taking the medication?
Most patients regain weight when they stop GLP-1 medications — this is a hard truth. The medication treats the condition (obesity); it doesn't cure it. Lifestyle changes during medication use can blunt regain, but most patients need long-term medication. We discuss this upfront.
How much weight can I expect to lose?
On semaglutide (Wegovy/Ozempic): average 15% body weight loss over 68 weeks in clinical trials. On tirzepatide (Mounjaro/Zepbound): average 20%+ over 72 weeks. Individual results vary significantly. We set realistic expectations during your first visit.
Do you do bariatric surgery referrals?
Yes — for patients with BMI ≥ 40 (or ≥ 35 with significant complications) where medication and lifestyle haven't worked, we refer to in-network NYC bariatric programs (NYC Health + Hospitals, NYU Langone, Mount Sinai, Maimonides) and provide pre/post-op medical support.
