Diabetes care that follows you between visits

Better control. Lower A1c. Less hospital.

The difference between a 7.5 A1c and a 6.8 A1c isn't usually about willpower or seeing your doctor more often — it's about having your numbers actually looked at, and your medication actually adjusted, when something needs to change. We use continuous data to drive continuous adjustments.

What we manage

  • Type 2 diabetes — most common, fully managed at CallMD Now
  • Type 1 diabetes — co-managed with your endocrinologist if you have one, or referred if you don't
  • Gestational diabetes — coordinated with your OB
  • Prediabetes — prevention program (see Diabetes Prevention)
  • LADA (latent autoimmune diabetes in adults) — diagnosis and co-management

Medications we manage

  • Metformin (first-line for most Type 2)
  • GLP-1 agonists (Ozempic, Trulicity, Mounjaro, Rybelsus)
  • SGLT2 inhibitors (Jardiance, Farxiga, Invokana)
  • DPP-4 inhibitors (Januvia, Tradjenta)
  • Sulfonylureas (when appropriate; older but inexpensive)
  • Insulin (basal and rapid-acting) — adjusted by us; pump and CGM support
What's included in diabetes care

Everything you need, integrated.

📡

Continuous Glucose Monitor (CGM)

Most NY Medicaid plans now cover CGMs (Dexcom, Libre) — game-changer for daily management. We help you get one.

💉

Insulin Optimization

We adjust basal and rapid-acting insulin based on your CGM data. Faster fine-tuning than waiting for quarterly visits.

🩺

Annual Eye/Foot Exams

We coordinate diabetic retinal screening (telemedicine retinal exam available) and refer to in-network podiatrists for foot exams.

🧪

Lab Monitoring

A1c every 3 months, kidney function, lipids, urine microalbumin — ordered to Quest/LabCorp on schedule.

🥗

Diabetes Nutrition Coach

Cultural-appropriate meal planning for NYC populations — Latino, Caribbean, South Asian, East Asian diets. Often $0 with Medicaid.

💊

Medication Cost Management

If a medication isn't covered, we find alternatives. We chase prior auths and use manufacturer programs when helpful.

FAQ

Common questions.

Can you prescribe Ozempic or Mounjaro?
Yes — for Type 2 diabetes, GLP-1 medications like Ozempic and Mounjaro are first-line for many patients and covered by most NY Medicaid Managed Care plans. Coverage for weight loss only (Wegovy, Zepbound) varies by plan. We verify coverage and handle prior auth before prescribing.
Will you order a CGM for me?
Yes — and we strongly recommend one if you're on insulin or have variable control. Most NY Medicaid Managed Care plans now cover Dexcom G7 or Libre 3 for Type 2 diabetes patients on multiple daily injections. We submit the order and handle insurance documentation.
What if I'm having a severe high or low blood sugar?
Severe hypoglycemia (low blood sugar with confusion, seizures, or unconsciousness) and severe hyperglycemia (DKA symptoms — nausea, vomiting, fast breathing, fruity breath) are emergencies. Call 911 or go to your nearest ER. For non-emergency concerns, message us — we respond same business day.
Do you handle insulin pump patients?
We co-manage pump patients with their endocrinologist or pump-trained specialist. For Type 2 patients new to pumping, we refer to a NYC endocrinologist for setup and partner on ongoing management.
What does diabetes care cost?
For NY Medicaid Managed Care members: visits $0, most medications $0–$3, CGM typically covered, lab work covered. For commercial insurance: visit copays vary; we verify before your appointment.

An A1c of 6.5 isn't a fantasy.

With the right care between visits, real change is achievable.
