We use your information to provide medical care, communicate with you, process insurance claims, comply with legal obligations, and improve our services. We do not sell your information. We do not use your medical information for advertising.
We share information only as needed: with your insurance company for billing, with your pharmacy for prescriptions, with labs for testing, with specialists when you're referred, and with our HIPAA-compliant technology vendors (under Business Associate Agreements). We share with law enforcement only when required by law.
You have the right to: see your medical records, request corrections, get a copy of who we've shared your information with, request restrictions on how we use your information, file a complaint without retaliation, and revoke any authorization at any time.
