Our pledge regarding your medical information

We understand that medical information about you is personal. We are committed to protecting it. This notice applies to all of the records of your care generated by CallMD Now.

How we may use and disclose your medical information

For Treatment: We may use and disclose your medical information to provide medical care. For example, your information may be shared between our providers, with consulting specialists, and with pharmacies for prescriptions.

For Payment: We may use and disclose your medical information to bill and collect payment from you, your insurance company, or another third party.

For Health Care Operations: We may use and disclose your information for our internal operations — quality improvement, staff training, licensing, and other administrative activities.

Other permitted disclosures

We may disclose your information without authorization in limited circumstances including: as required by law, for public health activities, to report abuse or neglect, for health oversight activities, for judicial proceedings, for law enforcement (only as required), for organ donation, for research (only with proper safeguards), to avert serious threat to health, and for specialized government functions.

Disclosures requiring your authorization

Most uses and disclosures of psychotherapy notes, uses for marketing purposes, and sales of your information require your written authorization. You may revoke any authorization at any time in writing.

Your rights regarding your medical information

• Right to inspect and copy: You may request access to your medical records. We will respond within 30 days.
• Right to amend: If you believe information is incorrect or incomplete, you may request an amendment.
• Right to an accounting of disclosures: You may request a list of certain disclosures we have made.
• Right to request restrictions: You may request restrictions on how we use or disclose your information.
• Right to confidential communications: You may request that we communicate with you in a specific way.
• Right to a paper copy of this notice: You may ask for a paper copy at any time.
• Right to file a complaint: If you believe your rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you.

Changes to this notice

We reserve the right to change this notice. The new notice will be effective for all medical information we maintain. The current notice will be posted on our website.
