Type 2 diabetes is one of the conditions telehealth manages best. We work with you on regular A1c monitoring (every 3-6 months, ordered at Quest or LabCorp), medication adjustments (metformin, GLP-1s like Ozempic or Wegovy when appropriate, insulin if needed), home blood sugar monitoring, dietary support, and connections to a registered dietitian. We coordinate with endocrinology for complex cases. What to have ready: your most recent A1c (if known), any home glucose readings, current medications, and what feels hardest about managing your diabetes.

High blood pressure

Hypertension is the most common chronic condition we treat — and one of the easiest to manage well by telehealth. We'll help you set up reliable home blood pressure monitoring, review your readings regularly, adjust medications as needed, and discuss lifestyle changes (salt, exercise, weight, sleep, alcohol) that move the needle. We coordinate referrals to cardiology if your blood pressure needs more aggressive management. What to have ready: a working home BP monitor (we can recommend brands), recent readings if you've been tracking, current medications, and any family history of heart disease.

High cholesterol is a major risk factor for heart attack and stroke — but with the right treatment, your risk drops significantly. We'll order lipid panels, calculate your 10-year cardiovascular risk, and discuss whether a statin is right for you. If you're already on a statin and tolerating it well, we'll keep an eye on your levels and side effects. If you've had problems with statins, we'll work with you on alternatives. What to have ready: your most recent lipid panel results (if known), any family history of heart disease, current medications, and any concerns about statins.

Medical weight management combines nutrition, exercise, behavior change, and (when appropriate) medications like semaglutide (Wegovy/Ozempic) or tirzepatide (Zepbound/Mounjaro). We evaluate whether GLP-1 medications are appropriate for you based on your weight, medical history, and insurance coverage. We coordinate with our registered dietitian for nutrition support. Insurance coverage for these medications varies — we'll help you navigate that. What to have ready: your weight history, any prior weight loss attempts, medical conditions, and what kind of support feels most helpful.

Asthma & COPD

Inhaler management, action plans, urgent flare-up help.

Asthma and COPD are well-managed by telehealth most of the time. We work on your daily inhaler regimen, write an asthma action plan you can keep on your fridge, manage flares before they become emergencies, and refer you to pulmonology if you need more advanced care. We can also help if you're trying to quit smoking — which makes a huge difference for COPD. What to have ready: your current inhalers (name and how often you use them), how often you have symptoms or use a rescue inhaler, and what tends to trigger your symptoms.
