Your annual physical is the foundation of good primary care. We'll review your full medical history, current medications, family history, lifestyle, and any concerns. We order age-appropriate screenings (blood work, cholesterol, diabetes screening, cancer screening referrals), assess your mental health, and coordinate any vaccines you need. While we can't do hands-on exams by video, telehealth annual visits work well for most healthy adults and we'll refer you for in-person care if anything needs it. What to have ready: a list of your current medications, any recent lab results, and a list of any concerns or symptoms you've had since your last visit.

Managing your medications is one of the most important parts of primary care. We handle refills of your existing prescriptions, dose adjustments when needed, and new prescriptions for new conditions. We also review your full medication list for any dangerous interactions and look for opportunities to simplify or reduce medications. If you're experiencing side effects, we'll work with you to either adjust the dose, switch medications, or find a better solution. What to have ready: your full medication list with doses, your pharmacy information, and any side effects or concerns you've noticed.

Lab work

Orders sent to Quest or LabCorp near you. Results reviewed by video.

We can order any blood test, urine test, or lab work that primary care typically needs. Orders go directly to Quest Diagnostics or LabCorp, with locations across all five boroughs and Long Island. Most results come back within 24-72 hours. Your provider will review them and message you with what they mean, or schedule a follow-up video visit to discuss in detail. Common labs include complete blood count, comprehensive metabolic panel, lipid panel, A1c, thyroid function, vitamin D, B12, and many others. What to have ready: your insurance card, and a Quest or LabCorp location you can get to easily.

Preventive care is the part of medicine that catches problems before they become serious. We'll review what screenings you're due for based on your age, sex, and risk factors. This typically includes cholesterol checks, diabetes screening, blood pressure monitoring, cancer screening referrals (mammogram, colonoscopy, Pap smear, prostate), and vaccines (flu, COVID, shingles, pneumonia, others). We coordinate referrals to NYC Health + Hospitals or other in-network locations for anything in-person. What to have ready: the date of your last physical, any family history of cancer or chronic disease, and your vaccine record if available.

When you need a specialist, we'll refer you to the right place — and we'll call ahead so you're expected. We have direct referral relationships with NYU Langone, Mount Sinai, Northwell, NYC Health + Hospitals, and major NYC specialty practices. We can refer to cardiology, gastroenterology, neurology, orthopedics, OB/GYN, pain management, and many other specialties. We'll also send your medical records so the specialist has everything they need before your visit. What to have ready: a description of the issue you need the specialist for, any tests you've already had done, and your preferred location or hospital network.

If you have a chronic condition like diabetes, high blood pressure, or high cholesterol, you may see specialists for the deep dives — but your primary care provider keeps everything coordinated in between. We monitor your medications, review lab results, check in on symptoms, and adjust your care plan as needed. We also keep an eye on the whole picture — like how your diabetes medication might affect your blood pressure or kidney function. What to have ready: the names of any specialists you see, your current medications, and any home monitoring data (BP readings, glucose logs, weight).

Maybe you've been more tired than usual, or your stomach has been off for a week, or you have a persistent symptom that's worth a conversation. Sick visits with your primary care provider are perfect for these in-between situations. We'll take a thorough history, order any tests we need, and either treat you, refer you, or watch and wait. What to have ready: a clear description of what's going on, when it started, and what makes it better or worse.

We can complete most medical forms you need: school physicals (for adults — for children see pediatrics), work return-to-duty notes, FMLA paperwork, short-term disability forms, jury duty exemptions, and DMV physical forms. Most forms can be completed during or shortly after a visit. Some forms require additional documentation or testing first — we'll let you know if so. What to have ready: the actual form (you can email or upload it before your visit), your medical history relevant to the form, and a clear sense of what the form is being used for.
