For Treatment: We may use and disclose your medical information to provide medical care. For example, your information may be shared between our providers, with consulting specialists, and with pharmacies for prescriptions.
For Payment: We may use and disclose your medical information to bill and collect payment from you, your insurance company, or another third party.
For Health Care Operations: We may use and disclose your information for our internal operations — quality improvement, staff training, licensing, and other administrative activities.
We may disclose your information without authorization in limited circumstances including: as required by law, for public health activities, to report abuse or neglect, for health oversight activities, for judicial proceedings, for law enforcement (only as required), for organ donation, for research (only with proper safeguards), to avert serious threat to health, and for specialized government functions.
Most uses and disclosures of psychotherapy notes, uses for marketing purposes, and sales of your information require your written authorization. You may revoke any authorization at any time in writing.
